Forms
The NCSSM CARE Network Report form may be used to submit concerns about students' emotional or physical well-being. When completing this form, please be as through, specific, and objective as possible about the situation you are reporting, so that we can best determine an appropriate response.
The NCSSM Incident Report form may be used to submit any concerns related to the NCSSM campus, our students, or our staff. When completing this form, please be as through, specific, and objective as possible about the situation you are reporting, so that we can best determine an appropriate response.
Please use this form if you are requesting approval to leave campus with an adult who is NOT your legal guardian AND if you will be missing curfew and/or be off campus for the night. You must receive approval prior to leaving campus. Please submit the form two days prior to departure.
